Making an Appointment
Please call the Capitol Surgeons at 301-588-0057 in order to make an appointment.
What You’ll Need
Please arrive at least 15 minutes prior to your appointment and bring:
- Identification (government-issued, such as a driver’s license)
- Insurance card
Your surgeon will need complete information about your condition, so be sure to also bring of these items that you have:
- Test results
- X-rays
- Mammograms
- Ultrasound
- Any other clinical information from your doctor or the emergency room
To save time at your first appointment, you can complete the Registration Form before your scheduled appointment. Please bring the form with you or email it to capitolsurgeons@hotmail.com

- CAPS Notice of Privacy Practice (for your reference)
- CAPS Medical Release Form
Use Adobe Reader to complete and email forms. Or print it and bring the completed form to your next visit.
Cancellations
We request 24 hours’ notice if you need to cancel so that we may accommodate other patients.
Your Other Doctors
Capitol Surgeons will keep your primary care and/or referring doctor apprised of your condition and treatment.
